Childbirth Class Reimbursements
Applying for reimbursement is easier than opening a door with a child safety lock.
- Complete the childbirth class reimbursement form
- Attach your proof of payment and completion certification
- Mail everything to Blue Cross Blue Shield at PO BOX 986030, Boston MA 02298
You must complete the course in order to be reimbursed, except when your delivery occurs before the course ends.
