Harvard University Health Services Support-to-Quit program

The Harvard University Health Services Support-to-Quit program provides resources and counseling to help you quit smoking. To get started on the program, contact your Harvard University Health Services primary care physician to obtain the necessary form to receive your free nicotine patches. For HUGHP members without a Harvard University Health Services primary care physician, please have your primary care physician fill out this form, and bring it to the Harvard University Health Services Pharmacy. Your primary care physician may also recommend a prescription for a smoking cessation medication that you can obtain for the lowest copayment available.

Living Healthy Smoke Free
